AZ MHC LLC d/b/a Anxiety Arizona (“Anxiety Arizona,” “we,” “us,” or “our”) respects your privacy and is committed to protecting information associated with your use of AnxietyArizona.com.

This Privacy Policy explains the information that may be collected when you visit our public website, how that information may be used, the technologies and third-party services associated with the website, and the choices available to you.

Anxiety Arizona is an Arizona telehealth psychology group practice providing outpatient psychological services to adolescents ages 13+, adults, and older adults throughout Arizona.

1. Scope of This Privacy Policy

This Privacy Policy applies to the public website located at AnxietyArizona.com and information collected through or in connection with your use of that website.

This Privacy Policy is separate from Anxiety Arizona’s Notice of Privacy Practices (HIPAA NPP).

Our Notice of Privacy Practices explains how Anxiety Arizona may use and disclose protected health information (“PHI”) in connection with health care services and describes your rights regarding that information.

When information constitutes PHI under the Health Insurance Portability and Accountability Act (“HIPAA”), applicable HIPAA requirements and our Notice of Privacy Practices govern our handling of that information.

2. The Public Website Is Not a Patient Portal

AnxietyArizona.com is a public informational website. It is not our electronic health record, patient portal, telehealth platform, or secure clinical messaging system.

The public website does not provide a client login or client account directly through WordPress.

Returning-client portal functions, clinical scheduling, clinical documentation, telehealth services, and other client functions are provided through our designated secure systems.

Please do not submit confidential medical, psychological, diagnostic, treatment, insurance, or other sensitive health information through ordinary website functionality or unsecured communication methods.

3. Information You Provide to Us

The public Anxiety Arizona website is intentionally designed to minimize the collection of personal and health information.

Scheduling and client-management functions are handled through our designated client systems.

3.3 No insurance information collected through the public website

The public website does not directly collect insurance member IDs, policy information, diagnoses, dates of birth, or other information for insurance verification.

Visitors seeking assistance with insurance verification are directed to designated communication methods.

3.4 No payment information collected through the public website

AnxietyArizona.com does not directly process or store credit-card or other payment information through the public WordPress website.

Client billing and payment functions are handled through designated practice systems.

3.5 No public website accounts

Visitors do not create Anxiety Arizona patient accounts directly through the public WordPress website.

Links labeled for existing or returning clients may direct users to an external secure client portal.

The fact that Anxiety Arizona advertises on a third-party platform does not mean that the platform’s advertising pixel or tracking tag is installed on AnxietyArizona.com.

At the time of this Privacy Policy, Anxiety Arizona does not intentionally use advertising pixels or website advertising/remarketing tags from Meta, LinkedIn, TikTok, or Pinterest on AnxietyArizona.com, and we do not intentionally use a Google Ads conversion or remarketing tag on the public website.

Our current Google Analytics configuration has advertising personalization disabled.

If our website technology practices materially change, we may update this Privacy Policy and our cookie disclosures accordingly.

8. Sensitive Health Information and Website Analytics

Anxiety Arizona recognizes that information relating to mental health and health care can be highly sensitive.

We have therefore designed the public website to reduce the opportunity for visitors to enter clinical or other sensitive health information into website fields that could interact with website analytics.

The public website does not currently provide a symptom checker, public appointment form, insurance form, patient-registration form, clinical intake form, or public client account through WordPress.

Clinical information should instead be provided through the practice’s designated systems and communication methods.

Website cookie consent is a website privacy choice. It is not a HIPAA authorization for the disclosure of protected health information.

9. Sessions Health

Anxiety Arizona uses Sessions Health for functions associated with client care and practice administration, which may include electronic health records, the client portal, scheduling, telehealth, documentation, billing, and related functions.

When you follow a link from AnxietyArizona.com to Sessions Health, you are leaving the public Anxiety Arizona website and interacting with a separate platform.

Information submitted through the client platform is handled according to applicable privacy and security requirements, contractual protections, and the policies applicable to that service.

Information constituting PHI remains subject to applicable health-information privacy requirements and Anxiety Arizona’s Notice of Privacy Practices.

10. Spruce

Anxiety Arizona uses Spruce for secure practice communications.

Visitors may be directed from AnxietyArizona.com to Spruce when they need to communicate with the practice, including when communication may involve information that should not be submitted through ordinary public website functionality.

Information communicated through Spruce in connection with health care may constitute PHI and is handled in accordance with applicable health-information privacy requirements and our Notice of Privacy Practices.

11. WordPress and Website Service Providers

AnxietyArizona.com is built and operated using WordPress and related website technologies and service providers.

These technologies may process technical information necessary to host, deliver, secure, maintain, or operate the website.

We may also use service providers to assist with website hosting, maintenance, security, analytics, consent management, technical support, and other operational functions.

Where a service provider creates, receives, maintains, or transmits PHI on behalf of Anxiety Arizona and qualifies as a business associate under HIPAA, we address that relationship as required by applicable law.

AnxietyArizona.com may contain links to Anxiety Arizona profiles or pages on third-party social-media platforms, including Facebook, Instagram, LinkedIn, TikTok, and Pinterest.

The website may also contain links to other third-party websites or services.

When you leave AnxietyArizona.com, the privacy practices of the destination website or service apply. Anxiety Arizona does not control the privacy practices of independent third-party websites.

Anxiety Arizona provides clinical services to adolescents ages 13 and older, adults, and older adults.

A parent, legal guardian, or other legally authorized adult generally establishes services with Anxiety Arizona for a client under age 18.

The public website is informational and does not intentionally solicit personal information from children through public forms, public accounts, newsletter registrations, or similar website features.

Our Google Analytics property is currently configured with limited retention settings.

Anxiety Arizona uses administrative, technical, and organizational measures designed to protect information appropriate to the nature of the information and the systems involved.

No website, internet transmission, electronic communication method, or information system can be guaranteed to be completely secure. Accordingly, we cannot guarantee absolute security.

We intentionally separate public website functionality from systems used to communicate and maintain sensitive clinical information.
