Welcome to AnxietyArizona.com. This website is owned and operated by AZ MHC LLC d/b/a Anxiety Arizona (“Anxiety Arizona,” “we,” “us,” or “our”).

These Terms of Use govern your access to and use of AnxietyArizona.com and the information, materials, resources, links, and other content available through this public website.

By accessing or using this website, you acknowledge these Terms of Use. If you do not agree with these Terms, please discontinue use of the website.

1. About Anxiety Arizona

Anxiety Arizona is an Arizona-based telehealth psychology group practice providing outpatient psychological services to adolescents ages 13+, adults, and older adults.

Clinical services are provided by licensed psychologists through telehealth to individuals located in Arizona, subject to clinical appropriateness, provider availability, applicable law, and the practice’s policies.

The public website provides information about Anxiety Arizona, anxiety and related concerns, psychological treatment, our services, and ways to contact or establish services with the practice.

2. Website Information Is for Educational and Informational Purposes

The information available on AnxietyArizona.com is provided for general educational and informational purposes.

Website content is not intended to replace individualized evaluation, diagnosis, psychological treatment, medical care, or advice from a qualified health care professional who is familiar with your individual circumstances.

Information about anxiety disorders, symptoms, diagnoses, psychological treatments, research findings, or other mental health topics is general information and should not be interpreted as a diagnosis, treatment recommendation, or individualized professional opinion.

Every person’s circumstances are different. Decisions concerning your mental or physical health should be made in consultation with appropriately qualified health care professionals.

3. Website Use Does Not Establish a Psychologist-Client Relationship

Visiting AnxietyArizona.com, reading information on the website, following Anxiety Arizona on social media, communicating with the practice about potential services, or using a link provided on the website does not by itself establish a psychologist-client or other professional treatment relationship with Anxiety Arizona or any Anxiety Arizona psychologist.

A psychologist-client relationship is established only after the practice has agreed to provide clinical services and applicable intake, consent, clinical, scheduling, and other requirements have been completed.

Submitting or communicating information to the practice does not guarantee that Anxiety Arizona or a particular psychologist will accept you as a client.

4. No Diagnosis or Treatment Through the Public Website

AnxietyArizona.com does not provide psychological diagnosis, psychotherapy, crisis assessment, or individualized treatment through its public webpages.

Descriptions of symptoms, disorders, treatment approaches, or research are not intended to determine whether you or another person has a particular diagnosis.

Only an appropriately qualified health care professional who has evaluated the individual can determine whether a diagnosis or particular treatment is appropriate.

5. Anxiety Research Articles and Educational Content

Anxiety Arizona may publish Anxiety Research Articles and other educational materials concerning anxiety, mental health, psychology, psychotherapy, and related subjects.

These materials are intended to make psychological information and research more understandable and accessible to the public. They are educational resources and are not individualized clinical advice.

References to scientific studies, professional literature, clinical guidelines, treatment approaches, or other sources do not mean that a particular finding or treatment applies to every person.

Psychological research evolves over time. Although we strive to provide useful and responsible information, website materials may not reflect every subsequent development in research, clinical practice, law, or professional guidance.

6. Evidence-Based Treatment Information

AnxietyArizona.com may describe evidence-based psychological treatments and approaches, including Cognitive Behavioral Therapy (CBT), Exposure and Response Prevention (ERP), Acceptance and Commitment Therapy (ACT), exposure-based interventions, and other psychological strategies.

The inclusion or description of a treatment approach on this website does not mean that the treatment is appropriate for every person or condition and does not guarantee a particular treatment outcome.

When Anxiety Arizona provides clinical services, treatment recommendations are based on the client’s individual circumstances, clinical needs, treatment goals, preferences, professional judgment, and other relevant factors.

7. No Guarantee of Treatment Results

Psychological treatment outcomes vary among individuals.

Anxiety Arizona does not guarantee that psychotherapy or any particular treatment approach will eliminate symptoms, produce a specific result, prevent recurrence, or achieve a particular outcome within a particular period of time.

Information describing the effectiveness of psychological treatments, including information derived from research studies, represents general research findings and does not constitute a promise or guarantee regarding an individual’s outcome.

8. Not an Emergency or Crisis Service

Anxiety Arizona is an outpatient telehealth psychology practice and does not provide emergency services, crisis intervention, 24-hour monitoring, or immediate-response care.

Do not use AnxietyArizona.com, email, website links, portal messages, text messages, or other routine practice communications to seek emergency assistance.

Messages sent to Anxiety Arizona may not be reviewed immediately.

If you are experiencing an emergency, believe you may harm yourself or another person, or believe another person may be in immediate danger, call 911 or go to the nearest emergency department.

You may also call or text 988 to reach the 988 Suicide & Crisis Lifeline.

Additional Arizona and national crisis resources are available on our Crisis Resources page.

9. Telehealth Services

Anxiety Arizona provides clinical services through telehealth.

Information on this public website about telehealth does not itself constitute informed consent for telehealth treatment.

Clients receiving services from Anxiety Arizona are provided applicable clinical disclosures, informed-consent information, practice policies, and telehealth information separately as part of establishing and receiving care.

Telehealth may not be clinically appropriate for every individual or every circumstance. Anxiety Arizona may determine whether its services are appropriate based on clinical, professional, technological, geographic, legal, safety, or other relevant considerations.

10. Arizona Services

Anxiety Arizona is an Arizona-based practice, and its clinical services are intended for clients located in Arizona as permitted by applicable professional and legal requirements.

The fact that AnxietyArizona.com can be accessed from another state, territory, or country does not mean that Anxiety Arizona offers or is authorized to provide psychological services in that jurisdiction.

Website access does not create an offer to provide professional services in any jurisdiction where doing so would be prohibited or inconsistent with applicable professional requirements.

11. Adolescents and Minors

Anxiety Arizona provides clinical services to adolescents ages 13 and older.

For a client under age 18, a parent, legal guardian, or other legally authorized adult generally must establish services with the practice.

An adolescent’s ability to consent to treatment independently, the rights of parents or guardians, and confidentiality involving minor clients are governed by applicable law and the circumstances of treatment.

The public website is informational and is not intended to allow a minor to independently establish or purchase psychological services when authorization from a parent, guardian, or other legally authorized person is required.

12. Scheduling and Availability

Information on AnxietyArizona.com regarding services, psychologists, appointment availability, or scheduling does not guarantee that an appointment will be available or that Anxiety Arizona will be able to accept a particular individual for treatment.

Appointment availability may change.

Anxiety Arizona may determine that its services are not appropriate for a prospective or existing client and may recommend or provide referrals to another provider, level of care, or service when clinically or otherwise appropriate.

13. Insurance Information

Information on AnxietyArizona.com regarding insurance participation, insurance benefits, or payment is provided for general informational purposes.

Insurance coverage is determined by the applicable health plan and may depend on factors including eligibility, benefits, deductibles, copayments, coinsurance, authorization requirements, medical necessity requirements, network status, and the specific services provided.

Verification of insurance benefits is not a guarantee of payment by an insurance company.

Clients remain responsible for understanding their insurance benefits and for amounts assigned to them under their health plan and applicable financial agreements, subject to applicable law.

14. Fees and Good Faith Estimates

Information regarding fees or self-pay rates on AnxietyArizona.com may be updated from time to time.

Individuals who are uninsured or who do not intend to use insurance to pay for care may have rights under federal law to receive a Good Faith Estimate of expected charges.

Please review our Good Faith Estimate page for additional information.

A fee displayed on the public website does not replace an individualized Good Faith Estimate when one is required by applicable law.

15. Client Portal and Sessions Health

AnxietyArizona.com may provide links to Sessions Health, which Anxiety Arizona uses for functions associated with client care and practice administration, including client portal functions, scheduling, electronic records, telehealth, documentation, billing, and related services.

When you follow a link to Sessions Health, you are leaving the public Anxiety Arizona website and accessing a separate platform.

Use of the client portal and other clinical systems may be subject to additional agreements, policies, notices, consents, and terms applicable to those services.

These Website Terms of Use do not replace any clinical, financial, privacy, telehealth, informed-consent, or other agreement applicable to your care.

16. Secure Communications and Spruce

Anxiety Arizona uses Spruce for secure practice communications.

Links from AnxietyArizona.com may direct visitors or clients to Spruce or other designated communication systems.

Communication with Anxiety Arizona through Spruce or another designated system does not change the emergency limitations described in these Terms. Routine electronic communications should not be used when immediate or emergency assistance is required.

17. Privacy

Your use of AnxietyArizona.com is also subject to our Privacy Policy, which explains our website privacy practices.

Our Privacy Policy is separate from our Notice of Privacy Practices, which describes how protected health information may be used and disclosed in connection with health care and explains your HIPAA privacy rights.

Website cookie preferences and website privacy choices are not substitutes for a HIPAA authorization, informed consent for treatment, or other authorization that may be required for clinical information.

18. Electronic Communications

Electronic communications can involve privacy, security, delivery, and technological risks.

You should not send sensitive clinical or health information through unsecured communication methods when a designated secure communication method is available.

Electronic communications may be delayed, misdirected, intercepted, filtered as spam, or otherwise fail to reach the intended recipient.

Anxiety Arizona does not guarantee immediate receipt or response to electronic communications.

19. Third-Party Websites and Services

AnxietyArizona.com may contain links to third-party websites, services, platforms, research publications, government resources, professional organizations, social-media platforms, or other external resources.

External links are provided for informational purposes or convenience.

Unless expressly stated otherwise, the inclusion of a link does not mean that Anxiety Arizona controls, sponsors, endorses, guarantees, or assumes responsibility for the third party or its information, products, services, security, availability, or privacy practices.

When you leave AnxietyArizona.com, your use of the third-party website or service is governed by that third party’s terms, policies, and practices.

20. Social Media

Anxiety Arizona may maintain accounts or pages on third-party social-media platforms.

Following, viewing, liking, sharing, commenting on, or otherwise interacting with Anxiety Arizona through social media does not establish a psychologist-client relationship and is not a substitute for clinical communication or treatment.

Because social-media interactions may be public or visible to others, you should not disclose confidential clinical information through public comments, posts, reviews, or other public-facing social-media features.

Anxiety Arizona cannot control the privacy practices of third-party social-media platforms or the actions of other users of those platforms.

26. Limitation of Liability

To the extent permitted by applicable law, AZ MHC LLC d/b/a Anxiety Arizona and its owners, clinicians, employees, contractors, and representatives will not be liable for losses arising solely from reliance on general public website information, inability to access the public website, or use of independent third-party websites or services linked from AnxietyArizona.com.

This limitation does not exclude or limit liability that cannot legally be excluded or limited and does not alter any professional obligations Anxiety Arizona or its psychologists owe to established clients under applicable law.

27. No Waiver of Client or Patient Rights

These Terms of Use govern the public website.

Nothing in these Terms is intended to waive, restrict, or eliminate any right that a client or patient has under HIPAA, applicable Arizona law, professional licensing requirements, insurance law, consumer-protection law, or another law that cannot legally be waived.

If you become a client of Anxiety Arizona, your clinical relationship is also governed by applicable informed-consent documents, practice policies, privacy notices, financial agreements, and applicable law.

28. Governing Law

These Terms of Use and use of the public AnxietyArizona.com website are governed by applicable federal law and the laws of the State of Arizona, without regard to conflict-of-law principles, except where another law necessarily applies.
